How We Protect Patient Information

R.A.C.H.F Diagnostic Laboratory protects patient information under Philippine privacy rules, including the Data Privacy Act of 2012. Health records, appointment details, contact information, request forms, and laboratory results are treated as confidential and sensitive personal information.

Information We Collect

We collect only the details needed for account access, appointment booking, queue updates, laboratory processing, result release, patient support, and required clinic records.

How We Use Information

Patient information is used only for clinic services, required recordkeeping, safety, lawful requests, and approved communication between the patient and the laboratory.

Access and Confidentiality

Only authorized clinic staff may access patient records. Staff must use patient information only for legitimate clinic work and must not share laboratory results or request forms with other people unless the patient gives consent or the law allows or requires it.

Patient Responsibilities

Patients should provide true and accurate information, keep passwords private, log out after using a shared device, and report suspicious account activity to the clinic.

Patient Rights

Patients may ask to review, correct, or update their account and appointment information. Patients may also ask questions about how their information is handled by the clinic.

By logging in or registering, the patient confirms that they understand this policy and agree that their information will be used for clinic services.
